The system of financing hospital care in compulsory health insurance in recent years faces many challenges. A current topic today is the high indebtedness of state and public hospitals. It is believed that the main reason for this is the insufficiently effective management of state and municipal hospitals. Although annual increases are being made each year to finance hospitals from the NHIF budget, the reported end-user satisfaction results are unsatisfactory. Another reason for the difficulties in the sector is the constantly rising prices of health services and the introduction of new expensive technologies. Factors related to the characteristics of the medical services market and the demographic development of the country contribute to the inefficiency. In this regard, the necessity of reforms in the system of financing of hospital care in Bulgaria is urgent. The purpose of the report is to provide some guidelines for improving the financial relationship between the only health insurance fund in Bulgaria and the hospitals in the country. By launching privatization in the sector, overall digital transformations and enhanced financial control will prevent the outflow of cash flows from unsatisfactory practices. Dealing with demographic conditions necessitates a reduction in the bed base. Changes in the normative documents and accreditation requirements and access to funding by the NHIF will help to improve the financial situation of the hospitals in Bulgaria
